MedExcel USA, Inc is currently in the development phase of an Aftercare Navigation Program. The program will help to better utilize healthcare resources in an efficient and cost effective model. Thus reducing redundancies in services and unnecessary readmissions to the hospital. This program will have significant benefits in a risk sharing environment.
The process will include working in conjunction with area facilities, by process of a call center, for follow up care after discharge to minimize return emergency department visits and readmissions. Working with catchment area affiliates, this process will assist to better serve the communities, while providing a service for continuum of patient care.
